What is body botox?
Body botox uses botulinum toxin to relax a large muscle so it gradually shrinks. In Korea the most requested areas are trap botox, for a longer neck and smoother shoulder line, and calf botox, for slimmer lower legs. Change builds over 2–4 weeks and lasts months. These are off-label uses in the US.
What can body botox change, and what can’t it?
Botulinum toxin blocks the signal from nerve to muscle. A muscle that works less gets smaller over the following weeks. That is the whole mechanism, so body botox only helps when the bulk you see is muscle.
- It can: soften a bulky upper trapezius that makes the neck look short, and slim calves that look wide because of a well-developed gastrocnemius muscle.
- It can’t: remove fat, tighten skin or change bone. If the fullness is mostly fat, liposuction or other body contouring is the more honest option.
A quick check: tense the muscle. If the shape gets noticeably bigger and firmer, there is muscle to work with.
Trap, shoulder and calf botox compared
| Trap or shoulder botox | Calf botox | |
|---|---|---|
| Muscle treated | Upper trapezius, between neck and shoulder | Gastrocnemius, the bulky upper calf muscle |
| Aim | Longer-looking neck, smoother shoulder slope | Slimmer lower legs |
| What studies measured | Smaller muscle area and thickness at 1, 3 and 6 months | Smaller calf size at 1–3 months |
| Main risk | Neck or shoulder weakness | Calf fatigue or weakness with walking and running |
| US FDA status | Off-label | Off-label |
Is body botox FDA-approved?
No. In the US, Botox Cosmetic is approved for four areas only: frown lines, crow’s feet, forehead lines and vertical neck bands. Trap botox and calf botox for shape are not on that list, so they are off-label. A licensed doctor can legally use a drug off-label, but no regulator has reviewed the evidence for these uses. The Botox label itself warns that serious side effects have been reported with unapproved uses, and that their safety and effectiveness have not been established.
The evidence that does exist is small. A 2025 study of 22 trapezius patients measured an area reduction of about 27–29% that held at six months. Published calf trials have been small too, with 20 to 40 women, and most followed people for weeks to a few months.
Why is body botox so popular in Korea?
A smooth shoulder line and slim lower legs are popular aesthetic goals in Korea. Korean researchers have published anatomy studies that map where the nerves enter the trapezius, so doctors can choose injection points more precisely. Several calf studies have come from Korean researchers too. As a result, trap botox and calf botox are routine menu items in Seoul clinics, not a niche request.
Who suits it, and who should avoid it?
It tends to suit people whose neck or calf bulk is clearly muscle, who want a subtle change, and who accept that it wears off.
Avoid it, or talk it through first, if you:
- Are pregnant or breastfeeding. There is no adequate safety data.
- Have a nerve or muscle disorder such as myasthenia gravis, Lambert-Eaton syndrome or ALS
- Have any trouble swallowing or breathing, or heart disease
- Have an infection or skin problem where the injections would go
- Have ever reacted to a botulinum toxin product
- Take aminoglycoside antibiotics or muscle relaxants, which can strengthen the toxin’s effect. Mention blood thinners and supplements too.
- Run, hike or play sport seriously (calf), or rely on shoulder strength for work or sport (trap). Talk through the trade-off with the doctor first.
How is it done in Seoul?
The doctor examines the muscle while you tense and relax it, and marks the injection points. Some clinics use ultrasound to check the muscle’s depth. The toxin is then injected at several points into the thickest part of the muscle. It takes minutes, and you walk out.
Doses are much higher than for the face. A review of body uses describes roughly 30 to 200 units per calf, depending on muscle size. Units are not interchangeable between brands, so a unit count only means something next to the brand name. If you’re also having face treatments the same week, ask for the total units across every area. The Botox label warns against exceeding the maximum total dose in one session.
How long does body botox last, and how does it fit a trip?
Muscle shrinks slowly. Expect the change to start showing over 2–4 weeks, with studies measuring a clear difference at one month. Trapezius studies have tracked results to six months. Calf studies so far have shorter follow-up, so ask the clinic how long they usually see it last and when they suggest a repeat.
On a 7–10 day trip you will fly home before you see the full result. That’s fine, because there’s nothing to recover from. Have it early in your stay, so the clinic can review you if something feels off before you leave.
Side effects and risks
- Soreness, redness and bruising at the injection points
- Trap: neck weakness if toxin reaches muscles closer to the neck, and weaker shoulder movement if it reaches the deltoid. Shoulder muscle can also thin with repeated sessions.
- Calf: at higher doses, weakness, a change in gait and fatigue after walking or running. A 2026 study found normal walking speed and step length, but subtle changes in balance control.
- Don’t massage the area, and wait two hours before a workout.
- Rarely, toxin effects spread beyond the injection area, hours to weeks later. Get urgent medical help if you have trouble swallowing, speaking or breathing.
What decides the price of body botox treatment in Seoul?
Body areas need far more toxin than the face, so units drive the price. What moves it: the toxin brand (imported or Korean-made), the total units per area, one area or both trap and calves, whether ultrasound guidance is used, and whether a follow-up check is included. Two quotes for “calf botox” can contain very different amounts of product. Ask for the brand and units per side in writing.

FAQ
How long does trap botox take to work?
The muscle shrinks gradually. Most people notice a change over two to four weeks, and studies have measured a clear reduction at one month.
How long does body botox last?
Months, not weeks. Trapezius studies have measured results lasting to six months. Calf studies have shorter follow-up, so ask the clinic when they usually suggest a repeat.
Will calf botox affect my running?
It can. Higher doses can cause calf weakness and fatigue, and one study found subtle changes in balance control even when walking looked normal. Tell the doctor if you run or play sport.
Does body botox get rid of fat?
No. It only shrinks muscle. If the fullness in your shoulders or calves is mostly fat, liposuction or other body contouring is the better fit.
Is trap botox FDA-approved?
No. Using botulinum toxin to reshape the trapezius or calves is off-label in the US. Botox Cosmetic is approved only for frown lines, crow’s feet, forehead lines and neck bands.
