How is eye bag removal done in Korea?

Most Seoul surgeons now reposition the under-eye fat rather than remove it. Through an incision inside the lower lid (transconjunctival), fat is moved down to fill the tear-trough groove, which flattens the bag and the shadow in one step. Removing fat alone can leave a hollow, tired look within a few years.

Three techniques, and which one fits your eyes

Technique Incision Best for Watch out for
Transconjunctival fat repositioning (“fat redistribution”, 눈밑지방재배치) Inside the lid, no visible scar Bags + dark groove, good skin elasticity; most patients under ~50 Needs a surgeon who does it routinely
Transconjunctival fat removal Inside the lid Pure bulging fat, no groove Can hollow out with age
Subciliary (external) lower bleph Just below the lashes Excess or crepey skin as well as fat; usually 50+ Lower-lid pull (ectropion) if too much skin is removed

The insider question: “Will you remove, reposition, or both, and why for my eyes?” A surgeon who offers only one technique to every patient is selling a technique, not a plan.

Is it eye bags, aegyo sal, or dark circles?

These get confused all the time and the fixes are completely different:

  • Eye bags: puffiness of fat below the lid, worse with age, visible even when you’re not smiling → surgery.
  • Aegyo sal: a small muscle roll directly under the lashes that shows when you smile. Many Korean patients want more of it. See aegyo sal vs eye bags.
  • Dark circles: pigment or thin skin, often with no bulge → non-surgical. See Korean dark circle treatments.

Good surgeons in Seoul often preserve the aegyo sal while removing the bag. Ask for this if you like yours.

What affects the cost of lower blepharoplasty in Seoul?

Korean quotes for lower blepharoplasty are set after an in-person or photo consultation, because the technique changes the price. What raises a quote:

  • External (subciliary) vs internal (transconjunctival) incision
  • Adding fat grafting to fill hollows
  • Adding a canthopexy to support the lower lid
  • General anesthesia instead of local with sedation
  • Combining with upper blepharoplasty in the same session

For a US benchmark, ASPS puts the 2024 surgeon fee for lower blepharoplasty at $3,709–$6,500, and upper blepharoplasty at $3,000–$5,500. Those are surgeon fees only; anesthesia and facility are billed separately. When you compare a Korean quote, ask for it itemised so you’re comparing the same things.

When filler or non-surgical treatment is enough

If the bag is small and the main issue is the groove, tear-trough filler can blend it for about a year. It’s not a fix for true fat bulges. Filler on top of a bag makes it look bigger. If you’re under 30 with mild puffiness that changes day to day, it’s often fluid or allergy, not fat. Check that before surgery.

Recovery: a 10-day Seoul plan

Day What to expect
0 Surgery under local + sedation, ~1–1.5 hours. Home or hotel the same day
1–3 Swelling and bruising peak. Cold compresses, head raised
4–5 Stitches out for external incisions (internal incisions dissolve)
7 Most people look “tired”, not “post-op”
7–10 Usual point to fly home
Weeks 4–12 Final settling; the lid shape matures

Risks to ask about

  • Hollowing from too much fat removal
  • Lower-lid malposition (pulling down, rounding of the corner), mostly with the external approach
  • Asymmetry and small residual bulges, sometimes needing a touch-up
  • Dry or irritated eyes for a few weeks

FAQ

Is eye bag removal permanent?

The fat removed or repositioned doesn’t come back, but skin and support tissue keep aging. Most patients get many years from it.

Will I have a visible scar?

Not with the transconjunctival approach. The incision is inside the eyelid. The external approach leaves a fine line just below the lashes that usually fades well.

Fat removal or fat repositioning: which is better?

For bags that sit above a dark groove, repositioning usually looks more natural over time. For pure bulges with no groove, removal can be enough.

How long should I stay in Korea for lower blepharoplasty?

About 7 to 10 days, which covers stitch removal and the main bruising.

Can I combine it with upper eyelid surgery?

Yes. Upper and lower blepharoplasty are commonly done in the same session, and the recovery stay is similar.
